AQSIQ证书申请企业提供资料目录
（1）《进口废物原料境外供货企业注册申请书》：需要法定代表人签名盖章。
（2）商业登记文件（复印件）：商业登记文件指申请企业的工商登记文件或税务登记文件，并附中文说明，说明登记证书种类、证书内容、发证机关、发证日期等。文件应当体现公司的法定代表人及公司注册地址。
（3）基本供货来源和环保质量控制措施的说明：基本供货来源指货物来源国别（地区）、主要上游加工回收生产厂名称、废物原料获取方式以及废物原料产生途径等；环保质量控制措施指给申请注册企业提供废物原料的生产、加工、回收厂的相关环境质量控制措施或管理制度，如公司简介，ISO9001或ISO14001证书，加工场地照片，加工流程，质量管理措施等。
（4）公司固定办公（仓库/加工场所）的平面图和照片：指固定办公场所（仓库/加工场所）标有尺寸和功能的平面图（A4纸）；照片分别体现公司固定办公（仓库/加工场所）大门（各2张）、全景（各3张）、局部（各2张），应当看到公司名称标牌。
（5）具体供货种类、数量：供货种类、数量指企业供货具体品种和年供货数量，货物的照片和说明；
（6）公司部门及人员：指公司划分的部门与人员人数、姓名、职责、学历等，负责各个质量控制环节技术人员姓名。
（7）质量控制管理制度（公司员工操作手册/质量管理文件）：应当包括
A、原料验收制度：指供货企业对收集或收购原料如何验收的规定或做法。包括进料流程、重量的确认、质量的确认、辐射的检测、实验室的检测、验收结果的评定制度等
B、生产加工过程质量控制：加工的流程及操作规程
C、危险夹带物及不可利用夹带物处理方式
D、最终产品检验制度
E、产品标识规定和方法
F、环保质量追踪制度和方法
G、检测设备或者委托的第三方检测机构或实验室
H、员工培训计划：指如何让员工了解、熟悉中国有关进口废物原料政策、掌握质量控制技能及货物鉴别技术等计划或做法。
I、不合格品控制的规定：指不合格品的规定及对不合格货物如何控制处理或处置的规定。包括进料、加工、出厂、装船、到港检验等各环节
如公司有ISO9001或ISO1400的证书可以不提供B-G文件
（8）计量、设备校准制度：计量器具设备名录、对计量器具设备校准、维护的周期规定等。包括生产、检验、包装的设备等，没有的可以不提供。
When applying to AQSIQ for provisional registration, the supplier enterprise shall submit the following materials:
1. Application Form for Provisional Registration of Overseas Supplier Enterprises of Imported Scrap Materials;
Note1: The form need to be sealed and signed by legal representative.
2. Business registration documents (copy);
Note2: It refers to business documents or tax affairs registration documents, all the documents need to be translated into Chinese ,there must some documents can show the legal representative’s name and the address of the company.
3. The basic source of the scrap and the description of the supplier’s quality control measures.
Note3: The basic source of the scrap means: which country (area) the scrap is from, the name of the previous company, how do you get the scrap, and how does the scrap come into being.
The quality control measures emphasize the measures or regulations of the suppliers on how to ensure the quality of the scrap meeting the requirement. For example: the supplier’s introduction, do they have certificate like ISO9001 or ISO14001, the photos of processing location, processing flow and quality control regulations.
4. The plan and some photos of the permanent plant (include warehouse and processing locations) and office
Note4: The size (length and width) of the permanent plant (include warehouse and processing locations) and office must be marked on the plan. The photos must show the company’s name, the gate, the warehouse, the processing locations, the facility, and so on. The more the better.
5. The type and quantity of scrap you will export to China each year;
Note5: you need to supply some photos of the scrap, and make a description of the scrap.
6. Organizational structure, responsibilities of each departments；
Note6: how many members in each department, and who takes responsible for each department. what responsibilities do they need to take?
7. Quality control management system (operating manual/quality control documents)
Note7: A Approval system of the scrap: the flow of purchasing the scrap, how to ensure the quantity, how to ensure the quality, how to examine the radiation, is there any standard to filtrate the qualified and unqualified scrap.
B Make a description of the processing flow and working instruction, and explain how to control the quality in this phase.
C How to handle with the dangerous carried- waste
D How to ensure the final products meeting the requirement.
E How to mark the scrap
F Environmental protection quality tracing system and method
G Is there any check-out facility, if not, consign which agency or the third party to check?
H The staff training: program: It refers that do you have learning schedule on how to let the staff understand and familiar with Chinese related policy, quality control system.
I How to dispose of the scrap failing to meet the quality requirements (from purchasing, processing, to shipment)
If you have certificate like ISO9001 or ISO14001, you don’t need to supply the documents from B to G.
8. Calibration system of measuring equipment
Note8: what measuring equipment do you have, how to calibrate them, how long to calibrate them once.
